Withdrawal Process
If any parent/guardian wishes to withdraw their child’s consent from the nasal flu vaccination, please fill in a withdrawal of consent form and return them back to school prior to the vaccination session. It is extremely important this form is completed in full and returned to the school prior to the immunisation session. Incomplete/inaccurate forms may result in your child being vaccinated.
This is the only way that the parents/guardians of the children will be able to withdraw consent. This will ensure that any errors on the day of the vaccination session are kept to a minimum.
